I'm not sure about the surname...never heard of it before. Maybe it's a given name, but I don't know how to pronounce it.

The first two lines say:
"I've gotten used to living in Tokyo, but all of a sudden miss my hometown"

The second two are a bit above me, but I think the third line has to do with thinking about being far away (or what is far away).

The last line, sou yuu mono, means "that kind of thing," but it's kind of a question, like, "is that the kind of thing?"
